To obtain an appointment at Premier Rheumatology of Oklahoma, we require a formal outside referral from a primary care provider or specialist; however, all patients must have an established primary care provider. Records required before an appointment can be scheduled include:
- Front and back copy of all current insurance cards
- Most recent office notes indicating reason for referral
- Any lab results supporting reason for referral
- Any imaging (MRI, CT, X-ray, or ultrasound) done in the last 10 years
- Medication list
- Patient demographics and contact information
Once these items have been received and determined to support a possible rheumatologic condition, the patient will be contacted to schedule an appointment.
Accepting most commercial insurance and traditional Medicare with a secondary supplemental plan.
